VICTORIAN HEALTH PROMOTION HUB

TERMS AND CONDITIONS

Members of the Victorian Health Promotion Hub (“the Hub”) agree to:

  1. ensure they have undertaken the necessary Police checks, Working with Children Checks and that their vaccinations are up to date in line with Victorian State Government requirements;

  2. adhere to VicHealth’s COVIDSafe Plan as per the Victorian State Government requirements;

2a) strongly encourage users to maintain up-to-date vaccination against COVID-19 as per Victorian State Government guidelines;

2b) ensure that no user attends the Hub if they are or feel unwell, or if they have any symptoms of illness;

  1. advise VicHealth immediately if they test positive to COVID-19 and were present in the Hub during the infectious period (which is 48 hours before symptoms appearing or 48 hours before the day they were tested) along with the names of any VicHealth staff members who were close workplace contacts;

  2. act in a safe, respectful and lawful manner and comply with all reasonable directions by VicHealth while visiting the Hub;

  3. keep safe and return any access device loaned by VicHealth while visiting the Hub;

  4. not to take, copy or use any information or Intellectual property belonging to VicHealth, other Hub members or VicHealth guests, including personal or confidential Information, trademarks and logos;

  5. protect the facilities and assets of VicHealth and other Hub members;

  6. be respectful in dealing with VicHealth staff, other Hub members, guests and contractors;

  7. not engage in any activity that is reasonably likely to be disruptive, damaging or dangerous to VicHealth, its employees or agents, other members, any other guests or any other third parties or property;

  8. ensure you have booked any resources that you will be using each day;

  9. register all their visitors and supervise their conduct for the period those visitors are using the Hub;

  10. act in a safe and lawful manner and comply with all reasonable directions by VicHealth while visiting the Hub;

  11. comply with the Occupational Health and Safety Act 2004 and any applicable regulations made under that Act;

  12. undergo any induction process as required by VicHealth prior to, or on the occasion of their first visit to the Hub;

  13. VicHealth does not grant exclusive access to the Hub and cannot guarantee availability of the location(s) or facilities;

  14. VicHealth may at any time in its sole discretion decline or terminate access to the Hub to any Hub member or visitor;
